VIOS error from physical NPIV FC adapter: FCA_ERR6 with error code "0000 0E6C"

Troubleshooting

Problem

Virtual FC client adapter on IBM i client is in inoperative state or LPM validation fails.

Symptom

A) Virtual FC client adapter on IBM i client is in inoperative state.
C) On the Virtual I/O Server side:
     1) The corresponding virtual FC server adapter (vfchost) status is "NOT_LOGGED_IN".
     2) The physical FC adapter mapped to the vfchost is logging FCA_ERR6 errors.
     3) Other clients that use the same physical NPIV port (FC name:fcs# in lsmap output) are working fine.
Sample output from Virtual I/O Server:
$ lsmap -vadapter vfchost29 -npiv

Environment

VIOS 3.1
Client OS: IBM i or AIX

Diagnosing The Problem

You can download the summ utility available at IBM AIX Support Center Tools to decode the FCA_ERR6 error, and install it on an AIX host or Virtual I/O Server from the oem_setup_env shell.
The decoding translates to something like this:
Feb 26 15:56:56 fcs4      T FCA_ERR6           FDISC failed during NPIV setup for C05XXXXXXXXX00BC
Notice the client's virtual WWPN is included

Resolving The Problem

From the Detail Data, the error code is "0000 0E6C" indicates the NPIV client requested a Re-login to the fabric (FDISC) and that failed.
When a physical adapter logs in to the switch, it does a FLOGI first.
However, when a virtual adapter logs in to the switch, it does a FDISC. In this case, FDISC is not working for the virtual adapter. This error is logged when we receive an LS_RJT (Link Service Reject) for this FDISC (Discover Fabric Service Parameters), and this error indicates the switch was unable to perform this request.
The error is coming from the SAN switch.
Contact your local SAN Switch Support Representative to review the switch port configuration for the virtual WWPN in question.
