What problems can be faced during firmware upgrade and what is the rollback procedure if the problem occurs?

Problems and Best practices:
 
If there are no hardware issues on the unit and the process is followed correctly, there is a low probability of having issues. Issues related to bad network connectivity during the install have been seen but very rarely.
As a best practice  it is recommended to perform a hard reset at the end of the update (power off the unit, remove power cords for a few minutes, and power it on). With that, it forces to apply the update changes (some times there are components that require a reboot to apply the updates). 
Rollback Procedure:
You can always install the previous version of firmware which should be available on fix central.
