Resolving The Problem
Verify that db_install_dir and db_exec_file configuration parameters are set correctly on guard_tap.ini file. This can also be checked from the GUI > Administration Console > Local Taps > S-TAP Control > Inspection Engine. Verify the fields DB Install dir and Process Name.
If you made changes to the guard_tap.ini file, you need to restart the guard_tap process. If you made changes on the GUI, the guard_tap process is automatically restarted when saving the changes.

Restart the ORACLE listener.
The above solution is if you are consistently missing these information on the reports.
