Error description

• This APAR will add support to create a new secondary index.
  It will also add support of creating a virtual Image copy.
  It will also fix a problem when running ORF agains a secondary
  index with a sparse routine.

  1. Adding a new secondary index not working.
  New secondary index was not allowed, and a garbage message
  was displayed.
  2. Secondary indexes with sparse routine.
  Multiple secondary indexes share the same sparse routine.
  The DMBXMPRM was created, but pointed to the same XDFLD.
  3. Adding support for Virtual IC.

• 1. Adding a secondary index is now possible.  However, there
  are some restrictions:
  -  not more than 5 at a time
  -  no shared index
  -  new index must be registered
  -  dataset for new index must be allocated
  2. The XDFLD field has been corrected.
  3. Two new parameter have been created:
  -  IC.VIC(Y|N)       default "N"
  -  IC.VICDSN(dsname)   default "VIRT.IC"
  When IC.VIC(Y) is specified, all indexes will have a
  virtual IC.
