Error description

• A system abend 0C4 PIC 11 occurred in DSQFQDRM or DSQDUPNM
  after the APPLY of PK34089(UK19727,UK19728,UK19729,UK19730)
  when printing a FORM that contains a user-defined column.
  A "DSQ60300 FM error -Invalid source data type *** CMD=PRINT"
  can also occur.

  * PROBLEM DESCRIPTION: If PK34089/UK19729 are applied,         *
  *                      unpredictable errors can occur when     *
  *                      report results are printed using        *
  *                      a FORM that contains a user-defined     *
  *                      column.  Errors seen include an         *
  *                      ABENDS0C4 in DSQFQDRM or DSQDUPNM and   *
  *                      the error message                       *
  *                      "DSQ60300 FM error - Invalid source     *
  *                      data type. *** CMD=PRINT".  Any error   *
  *                      from PRINT REPORT using a FORM is a     *
  *                      possible candidate for this fix.        *

  * RECOMMENDATION:                                              *
  ****************************************************************
  The fix for PK34089 reinitialized some report flags to zero
  at the beginning of new-report processing in DSQFPREF.
  However, this code is executed twice for a single report if that
  report is printed.  Flags that were set when the report was
  first generated need to be still extant when that report is
  printed, or, for certain form configurations, error conditions
  can occur.
  

Problem conclusion

• The code that reinitializes form flags was removed from
  DSQFPREF.
