Error description
Multiple HTTP requests coming in simultaneously can result in multiple TAI object being instantiated when only one is needed. This can result in faulty TAI chains being created and several exceptions being thrown in the Servant Region.

Problem conclusion
Code has been updated so there is only one TAI object is created. APAR PI10840 is currently targeted for inclusion in Fix Packs 7.0.0.33, 8.0.0.9 and 8.5.5.3 of WebSphere Application Server.
