Error description

• Once an Internal Approval process is saved for an Environment,
  there is no way to delete it.
  
  Steps to recreate -
  1)  Create application
  2)  Create environment
  3)  Check "Require Approvals" in the Environment Configuration
  4)  Edit and save the internal approval process.
  5)  Uncheck "Require Approvals" in the Environment Configuration
  6)  Check "Require Approvals" in the Environment Configuration
  7)  The prior internal approval process still exists and was not
  deleted
