Error description
Clients upgrading from some older Application Discovery levels that used a predefined name (EZ#Annotatinos) are not given the needed steps to configure the existing database in AD Configuration Server during the upgrade.
Local fix
Locate/confirm existing database name (e.g. via SQL Server Management Studio) and add the Relational Database Server name which is installation-dependent, the database name (EZ#Annotations) and schema name (DBO) to the Annotations Database page under the Configuration in use under the Environment defined in Configuration Server's administrative gui.
