Error description

• During the editing of a QMF-FORM and displaying the report we
  get message:
    DSQ60291 FM error - failed to insert record into update
                        checkpoint index. *** CMD=DISPLAY

    To reproduce the problem using a simple query on Q.STAFF.
  .
  If you execute the query and make the following actions in the
  same sequence you get the error:
  - Execute this SQL-statement:

Problem summary

  * PROBLEM DESCRIPTION: User's modifying FORM options and       *
  *                      toggling back to a reformatted report   *
  *                      using SHOW REPORT or the PF12 REPORT    *
  *                      function key may receive error DSQ60291 *
  *                      'FM error - failed to insert record     *
  *                      into update checkpoint index.' Other    *
  *                      symptoms may include incorrect line     *
  *                      numbers and out of place END markers.   *

  * RECOMMENDATION:                                              *

  Module DSQFMCTL was incorrectly calculating the end of report
  line number when a recalculation due to a change in the number
  of variations was being performed on an existing report. If the
  updated form was saved and run with the query, the problem would
  not have occurred.
