Error description

BBGZSCFM modules entries in the LPA are not being cleaned up
correctly upon server shutdown.

Local fix

Remove duplicate entries:
1) Always use dynamic LPA DELETE with caution(!)
2) Dynamic LPA DELETE does distinguish a BYADDR case and is
smart enough not to delete the module in this case
We suggest trying to delete one entry and checking the results
before deleting all N-1 of them.

*                      entries for BBGZSCFM, BBGZAFSM and      *
*                      BBGZADRM.                               *

Output of the IPCS LPAMAP command shows extra entries for
BBGZSCFM, BBGZAFSM and BBGZADRM because the CSVDYLPA
REQUEST=DELETE service is being called incorrectly.  BBGZSCFM
entries added by calling CSVDYLPA REQUEST=ADD are not being
cleaned up correctly on server shutdown.  BBGZAFSM and BBGZADRM
entries added by calling CSVDYLPA REQUEST=ADD are not being
cleaned up correctly when new versions are loaded.

The extra entries do not effect the runtime and there are no
extra copies of BBGZSCFM, BBGZAFSM, and BBGZADRM in storage.
The call to CSVDYLPA REQUEST=DELETE for BBGZSCFM, BBGZAFSM and
BBGZADRM is using the wrong parameter list mapping and is not
setting the module name that is a required parameter.

Problem conclusion

Updated the code to call CSVDYLPA REQUEST=DELETE correctly for
BBGZSCFM, BBGZAFSM, and BBGZADRM.

The fix for this APAR is currently targeted for inclusion in fix
pack 18.0.0.3.  Please refer to the Recommended Updates page for
