Error description

• ISSUE: Duplicate inspection forms are associated with a Location
  
  
  DESCRIPTION: In Work Order Tracking, if the user specifies a
  location to the work order and specifies an inspection form to a
  task, duplicate inspection records are created and associated to
  the location.
  
  STEPS TO REPRODUCE:
  1. Login to Maximo
  2. Go to Work Orders | Work Order Tracking
  3. Click on New
  4. In the Location field, select a location
  5. Click on the Plans tab
  6. In the "Task for Work Order XXX" section, click New Row
  7. In the Inspection Form field, select an inspection form
  8. Click Save
  9. In the "Work Reference Information" section, click on
  Locations > Go To Locations
  10. In More Actions sections, click on View Work Details
  11. Click on the Inspections tab
  
  EXPECTED RESULT:
  This should display one inspection form
  
  ACTUAL RESULT:
  Two inspection forms are displayed
