IC90049: UNINTENDED SEGV IN SQAPI_WARN_SETUP() WITH UDR MI_EXCEPTION CALLBACK HANDLING

Error description

• If a user defined routine (UDR) with an mi_exception callback
  registered encounters an error trying to execute some SQL, but
  the error is not recorded correctly, a segmentation violation
  will occur in sqapi_warn_setup(). The following is an example of
