Solution
IBM strongly recommends applying MegaRAID Firmware image version 1.03.20-0400 or higher.

Additional information
When a logical volume is running a background I/O operation such as an "initialization" or a "consistency check", and the system is rebooted, there is a very low probability for the controller to acknowledge a complete I/O to the host operating system prior to the I/O being completed.
This is a rare issue can result in data corruption on the logical volume, and as a result, the user would see errors reported by the operating system or an application.
