Hazmat Manifest Layout

A Shipper's Declaration is auto generated when a shipment manifest contains hazardous materials.

Field Description
EMERGENCY CONTACT PHONE NUMBER

The phone number for the person to contact in the event of emergency. This is manually entered by the shipping supervisor.

FROM

The address from where the shipment originates.

DESCRIPTION OF MATERIAL

The description of the material being shipped.

HAZARD CLASS

The classification to which the hazardous material belongs.

PACKING GROUP

The packing group to which the material belongs.

L.D. NO.

The UN number of the shipment.

DOT LABEL CODE

The label code of the shipment as registered with the department of transportation.

WT./VOL

The weight per volume.

PRO #

The PRO number assigned to identify freight bills.

RECEIVER'S NAME DESTINATION-CITY

The name and the address of the receiver.

STATE

The state to which the shipment is shipped.

Shipper's Signature

The signature of the shipping supervisor.

Date

The date on which the shipper's declaration is signed by the shipping supervisor.
