Use data entry forms

You can use data entry forms to manually upload event records for a combination of event types, including originate, ship, receive, and transform. You can also view, edit, and resubmit event records that were created in the past 30 days to correct any errors in the data entry. Only events that are uploaded with data entry forms are displayed on the data entry forms page. Event records uploaded by other methods are not visible on the data entry forms page. Since IBM Food Trust uses Blockchain to store information, a complete history of all submitted event records, including resubmissions, is retained.

To get started, click the menu icon (). Then, click Data entry forms.

Record types

You can select the following record types:

Record types

You are provided with the following data entry forms:

  • Originate
    Use when products are originated, for example when a product is harvested or created. Submitting this record type creates a commission event.
  • Originate and ship
    Use when products are originated (created or harvested) and shipped to another location.
    • If you select Lot as the Shipment type, a commission event and an observation shipping event are created when you submit this record type.
    • If you select Container as the Shipment type, commission event and an aggregation event are created when you submit this record type.
  • Receive and record origin
    Use when products are received from another location and the product originate records were not previously recorded. Submitting this record type creates a commission event, an observation shipping event, and an observation receiving event.
  • Receive
    Use when products are received from another location and the product origination records were already recorded.
    • If you select Lot as the Shipment type, an observation receiving event is created when you submit this record type.
    • If you select Container as the Shipment type, a disaggregation event is created when you submit this record type.
  • Ship
    Use when products are shipped to another location.
    • If you select Lot as the Shipment type, an observation shipping event is created when you submit this record type.
    • If you select Container as the Shipment type, an aggregation event is created when you submit this record type.
  • Transform
    Use when one or more input products are transformed or relabeled into one or more output products. Submitting this record type creates a transformation event.

How to use the forms

You can use the data entry forms to manually create records or view records that were submitted within the past 30 days. Identify the record type that you are interested in, then:
  • To view recently submitted records, click View history. A list of available records opens.
    Note: Only event records that are uploaded by using data entry forms can be viewed on the Event Record page.

  • To create a record, click Create new record. A fillable form opens. See the event record types for details.
    Note: The event record time zone is based on your location, not the facility location.

Originate

Select Originate as the event record type when products are originated, for example when a product is harvested or created. Submitting this record type creates a commission event.
Note: This form is available to organizations without the FSMA compliance feature enabled.

Ensure you include the following information:

  • Originating details
    • Location. Select the location that the products originate from.
    • Date. Select the date that the products were originated.
    • Time. Select the time that the products were originated.
  • Product details
    • Product. Select the product that was originated.
    • Lot code. Enter the lot code for the product that was originated.
    • Quantity. Enter the quantity of product that was originated.
    • Units of measure. Select the unit that the products are being measured with.

Originate and ship

Select Originate and ship as the event record type when products are originated (created or harvested) and shipped to another location.

If you shipped a lot of products, select Lot as the Shipment type. A commission event and an observation shipping event are created when you submit this record.
Note: This form is available to organizations without the FSMA compliance feature enabled.

Ensure you include the following information:

  • Originating details
    • Location. Select the location that the products originate from.
    • Date. Select the date that the products were originated.
    • Time. Select the time that the products were originated.
  • Product details
    • Product. Select the product that was commissioned.
    • Lot code. Enter the lot code for the product that was commissioned.
    • Quantity. Enter the quantity of products that was commissioned.
    • Units of measure. Select the unit that the product is being measured with.
  • Shipping details
    • Destination location. Select the location that the products are shipped to. If you are not already entitled to this location, click Enter location ID and enter the GLN or IBM location identifier of the location.
    • Shipping date. Select the date that the products were shipped.
    • Shipping time. Select the time that the products were shipped.

If you shipped a container (SSCC), select Container as the Shipment type. A commission event and an aggregation event are created when you submit this record.

Ensure you include the following information:

  • Originating details
    • Location. Select the location that the products originate from.
    • Date. Select the date that the products were originated.
    • Time. Select the time that the products were originated.
  • Product details
    • Product. Select the product that was commissioned.
    • Lot code. Enter the lot code for the product that was commissioned.
    • Quantity. Enter the quantity of products that was commissioned.
    • Units of measure. Select the unit that the product is being measured with.
  • Shipping details
    • Container ID. Enter the SSCC or LPN identifier or scan the GS1-128 encoded SSCC-18 bar code.
    • Destination location. Select the location that the container was shipped to. If you are not already entitled to this location, click Enter location ID and enter the GLN or IBM location identifier of the location.
    • Shipping date. Select the date that the container was shipped.
    • Shipping time. Select the time that the container was shipped.

Receive and record origin

Select Receive and record origin as the event record type when products are received from another location and the product originate records have not been previously recorded. Submitting this record type creates a commission event, observation shipping event, and an observation receiving event.
Note: This form is available to organizations without the FSMA compliance feature enabled.

Ensure you include the following information:

  • Receiving details
    • Receiving location. Select the location where the products were received.
    • Receiving date. Select the date that the products were received.
    • Receiving time. Select the time that the products were received.
  • Originating details
    • Product. Select the product that was originated.
    • Lot code. Enter the lot code for the product that was originated.
    • Quantity. Enter the quantity of product that was originated.
    • Units of measure. Select the unit that the product is being measured with.
    • Location. Select the location that the product originated from.
    • Date. Select the date that the product was originated.
    • Time. Select the time that the product was originated.

Transform

Select Transform as the event record type when one or more input products are transformed or relabeled into one or more output products. Submitting this record type creates a transformation event.

Ensure you include the following information:

  • Transformation details
    • Location. Select the location where the products were produced.
    • Production date. Select the date that the products were produced.
    • Production time. Select the time that the products were produced.
  • Input product(s)
    • Product. Select the products that were input into the transformation.
    • Lot code. Enter the lot code for the products that were input into the transformation.
    • Quantity. Enter the quantity of products that were input into the transformation.
    • Units of measure. Select the unit that the input products are measured with.
  • Output product(s)
    • Product. Select the products that were output from the transformation.
    • Lot code. Enter the lot code for the products that were output from the transformation.
    • Quantity. Enter the quantity of products that were output from the transformation.
    • Units of measure. Select the unit that the output products are measured with.
